Medical Termination of Pregnancy (MTP) is much safer during the first trimester (up to 12 weeks), when the pregnancy can often be terminated using relatively simple procedures (e.g. vacuum aspiration/suction, or menstrual regulation), with minimal instrumentation and low risk of complications.

By the second trimester, the foetus is considerably larger, more developed, and more firmly implanted, and the uterus itself has undergone more physiological change. Terminating a pregnancy at this stage requires more invasive procedures (such as dilation and evacuation, or induced labour using drugs), which involve greater blood loss, longer procedure time, higher risk of uterine injury/perforation, haemorrhage, and infection.
