Retroperitoneal organs are positioned behind (dorsal to) the peritoneal cavity's lining, in contrast to intraperitoneal organs, which are wrapped by peritoneum and suspended by mesenteries.

The peritoneum is the serous membrane lining the abdominal cavity (parietal peritoneum) and covering most abdominal organs (visceral peritoneum), with a potential space (peritoneal cavity) between the two layers.

  • Intraperitoneal organs (e.g., stomach, spleen, most of the small intestine) are almost completely covered by visceral peritoneum and held in place by a mesentery, allowing some mobility.
  • Retroperitoneal organs lie behind (retro-) the peritoneum, against the posterior abdominal wall, with peritoneum covering only their anterior surface (or none at all). They are relatively fixed in position.

Examples of retroperitoneal organs: the two kidneys, the adrenal (suprarenal) glands, the ureters, most of the pancreas, the duodenum (2nd–4th parts), the abdominal aorta, and the inferior vena cava.
