Diabetes mellitus comes from an insulin deficiency; goitre and cretinism both come from a shortage of thyroid hormones, typically because the diet lacks iodine.

Many endocrine disorders appear when a gland fails to supply enough of its hormone. The three named here fall into two glands — the pancreas and the thyroid.

  • (a) Diabetes mellitus. Insulin, from the beta cells of the Islets of Langerhans, normally lowers blood glucose by moving it into cells and storing it as glycogen. When insulin is deficient (or the body cannot respond to it), glucose is not taken up and blood sugar stays high for long periods. Glucose then spills into the urine and harmful ketone bodies form. The condition is therefore a hormonal deficiency of insulin.
  • (b) Goitre. The thyroid needs dietary iodine to build its hormones T3 and T4. If iodine is lacking, hormone production falls and the gland enlarges in an attempt to compensate, producing the visible swelling of the neck called goitre. So goitre reflects a deficiency of thyroid hormone. …
