The title 'Birth' is justified on several levels at once. Most obviously, it refers to the literal birth of the Morgans' child during the course of the extract. But the word also describes something closer to a second birth, since the infant is born apparently lifeless and has to be coaxed into his first breath and cry through Andrew's prolonged effort, so that his real entry into life happens only after the delivery itself. At the same time, the ordeal marks a kind of birth for Andrew as a doctor: coming to the Morgans' in low spirits about his marriage and his practice, he leaves having rediscovered, through hard, selfless work, why he became a doctor in the first place.
