A drop in blood calcium disturbs the calcium-dependent switch that controls contraction, producing tetany, uncontrolled violent muscle spasms.

Calcium ions play a central, switch-like role in ordinary muscle contraction. In the resting state, the active sites on actin where myosin would bind are masked by troponin. Contraction becomes possible only when calcium ions, released from the sarcoplasmic reticulum after a nerve signal, bind to troponin and remove this masking, exposing the sites so that myosin can attach and pull.

Tetany occurs at the other extreme of calcium regulation: when the level of calcium ions in the body fluids falls too low, rather than triggering normal, controlled contraction, it produces the opposite, uncontrolled kind - the muscles begin to twitch and contract violently, and these rapid, wild spasms and cramps of the muscle are together called tetany. …
