Medical termination of pregnancy (MTP) is legally permitted in India when continuing the pregnancy poses a risk to the woman's physical or mental health, when the foetus has severe abnormalities, or when pregnancy results from contraceptive failure or rape.

The Medical Termination of Pregnancy Act, passed in 1971 and amended in subsequent years, recognizes that there are specific circumstances under which ending a pregnancy becomes medically necessary or ethically justified. The law was framed not only to protect women's health but also to curb unsafe, illegal abortions that were claiming thousands of lives.

The conditions under which MTP is advised fall into several broad categories, each grounded in either medical necessity or compassionate consideration of the woman's circumstances.

Health-related grounds form the primary justification. When continuation of the pregnancy would involve a risk to the life of the pregnant woman, or would cause grave injury to her physical or mental health, termination is advised and legally permitted. This recognizes that pregnancy, while natural, can sometimes threaten a woman's well-being in serious ways — conditions like severe heart disease, uncontrolled diabetes, or psychiatric illness may worsen dangerously during pregnancy.

Foetal abnormalities constitute another important ground. If there is substantial risk that the child, if born, would suffer from serious physical or mental abnormalities that would severely impair quality of life, MTP may be advised. Advances in prenatal diagnostic techniques now allow doctors to detect conditions like severe neural tube defects or chromosomal disorders early in pregnancy.
