The human immunodeficiency virus (HIV) is a fragile virus that cannot survive long outside the human body. It does not spread through air, water, or casual contact. Transmission requires the virus from an infected person to enter the bloodstream of another person. This happens only through certain body fluids that contain a high concentration of the virus: blood, semen, pre-seminal fluid, rectal fluids, vaginal fluids, and breast milk.

The NCERT textbook clearly outlines the major routes of transmission. Understanding these is crucial not just for exams but for dispelling myths and reducing stigma.

Sexual transmission is the most common route worldwide. Unprotected sexual intercourse — vaginal, anal, or oral — with an infected partner can transmit the virus. The virus in semen or vaginal secretions enters the body through tiny tears or directly through the mucous membranes lining the genitals, rectum, or mouth. Anal sex carries a higher risk because the rectal lining is delicate and more prone to injury.

Transmission through blood happens in several ways. Sharing of contaminated needles and syringes among intravenous drug users is a major route — a tiny drop of infected blood left inside a needle is enough. Receiving infected blood or blood products (like during transfusion) was a significant risk before mandatory screening began. Today, blood banks in India test all donated blood for HIV, making this route rare but not impossible if protocols fail.
