The sample project uses a fifteen-item questionnaire designed to capture exactly the information listed for the study — the respondent's background, their current toothpaste use, their preferences, and the media that influence them. Its questions are:
- Name
- Sex
- Ages of family members (in years)
- Total number of family members
- Monthly family income
- Location of residence — Urban / Rural
- Major occupation of the main bread-winner — (i) Service (ii) Professional (iii) Manufacturer (iv) Trader (v) Any other (specify)
- Does your family use toothpaste to clean your teeth? — Yes / No
- If Yes, the essential qualities of a good toothpaste (tick more than one) — (i) Plain (ii) Gel (iii) Antiseptic (iv) Flavoured (v) Caries protection (vi) Fluoride (vii) Other
- If Yes, which brand of toothpaste do you use?
- How many 100-gram packs of this toothpaste do you use per month?
- Are you satisfied with this toothpaste? — Yes / No
- Are you prepared to try a new toothpaste? — Yes / No
