The Female Athlete Triad is not three separate problems that happen to coexist. It is a single clinical syndrome in which the three components drive one another in a vicious cycle. Understanding the triad matters because it can quietly undermine an athlete’s health and performance long before any obvious injury or illness appears.
The core of the triad is low energy availability — not simply “not eating enough,” but a mismatch between the energy the athlete takes in through food and the energy she expends in training and daily life. When that gap becomes too large, the body perceives a state of energy crisis. To conserve fuel for essential functions, it begins to shut down non-urgent systems. The first system to be suppressed is often the reproductive axis, leading to menstrual dysfunction — irregular cycles, missed periods (amenorrhea), or anovulation. At the same time, low energy availability reduces the production of oestrogen, a hormone that normally protects bone. Without adequate oestrogen, bone remodelling tilts toward breakdown, and low bone mineral density develops. Over time, this can lead to stress fractures, premature osteopenia, or even osteoporosis in young women.
An athlete does not need to have all three components to be at risk. Even one or two of the triad conditions — for example, irregular periods with a history of stress fractures — warrants evaluation, because the other components may be developing silently. …
