The reproductive life of a human female is punctuated by two defining physiological milestones that together frame her fertile years. These are not merely biological switches but events with profound significance for her body, health, and life trajectory.

Menarche is the first menstrual flow, typically occurring between the ages of 10 and 15 years. Its significance lies in what it announces: the girl has entered puberty, and her reproductive system has matured to the point where cyclical ovulation can begin. The hypothalamus starts releasing GnRH (gonadotropin-releasing hormone), which triggers the pituitary to secrete FSH and LH, setting in motion the monthly ovarian and uterine cycles. Menarche does not guarantee immediate fertility—early cycles are often anovulatory—but it signals that the machinery is now in place. From this point forward, the uterus prepares itself every month for a possible pregnancy, shedding its lining when fertilization does not occur. Menarche thus marks the beginning of the reproductive phase of life.

Menopause, on the other hand, is the permanent cessation of menstruation, generally occurring around 45 to 50 years of age. It represents the end of a woman's reproductive capacity. The ovaries gradually lose their responsiveness to pituitary gonadotropins, follicles become depleted, and estrogen and progesterone levels decline sharply. Menstruation becomes irregular and eventually stops altogether. A woman is said to have reached menopause when she has gone without a menstrual period for twelve consecutive months.
