Q.In case of an infertile couple male has abnormally low sperm count in ejaculation. Also Female has blocked oviducts and has spermicidal secretions in vagina but can provide suitable environment for development of embryo. Suggest and explain the assisted reproductive technology (ART) useful to this couple.

With low sperm count in the male and blocked oviducts (plus a hostile vaginal environment) in the female, fertilisation must happen outside the body (IVF, ideally with ICSI) and the resulting embryo placed directly into her uterus, which can support development.

Analysing the couple's problems:

  • Male: Oligospermia (abnormally low sperm count) — natural fertilisation is unlikely because too few sperm reach the egg.
  • Female: Blocked oviducts (fallopian tubes) — sperm cannot travel up to meet the egg naturally, and a fertilised zygote formed in vivo could not travel down to the uterus either. Spermicidal vaginal secretions also kill sperm before they can even ascend. However, her uterus is capable of supporting embryo development.

Suitable ART: IVF with ICSI, followed by ET (zygote/embryo transfer into the uterus)

  1. Ova are collected from the female's ovaries (bypassing the blocked oviducts) after hormonal stimulation.
  2. Because the male's sperm count is too low for standard IVF (where thousands of sperm are simply mixed with the egg), ICSI (Intra-Cytoplasmic Sperm Injection) is used — a single healthy sperm is directly injected into the egg's cytoplasm to ensure fertilisation. …
