The foetal ejection reflex is a neuroendocrine positive feedback loop triggered when the fully developed foetus and placenta release oxytocin, causing uterine contractions that push the foetus against the cervix, which in turn stimulates more oxytocin release — amplifying contractions until delivery is complete.

Parturition, the act of giving birth, is not a sudden event but the culmination of a carefully orchestrated hormonal cascade that begins towards the end of pregnancy. After approximately nine months of gestation, signals from the fully developed foetus and the placenta initiate the process. The foetus itself plays an active role: it releases oxytocin, a peptide hormone, which acts on the mother's uterine muscles. Simultaneously, the placenta secretes oxytocin and other hormones that prime the uterus for labour.

Oxytocin is the key player here. When it reaches the myometrium — the smooth muscle layer of the uterus — it triggers mild contractions. These initial contractions are significant because they set in motion a remarkable positive feedback mechanism. As the uterus contracts, the foetus is gently pushed downward, and its head (or presenting part) begins to press against the cervix, the narrow neck of the uterus. This mechanical pressure is detected by stretch receptors in the cervical wall.

The stretch receptors send nerve signals to the mother's hypothalamus, which in turn commands the posterior pituitary gland to release even more oxytocin into the bloodstream. This additional oxytocin intensifies the uterine contractions, which push the foetus harder against the cervix, which stretches it further, which triggers yet more oxytocin release. This self-amplifying cycle is the foetal ejection reflex — a positive feedback loop that grows stronger with each iteration.
