Down syndrome results from the presence of an extra copy of chromosome 21 (trisomy 21) caused by non-disjunction during gamete formation, producing a characteristic set of physical and developmental features.

Cause: During meiosis, chromosome 21 sometimes fails to separate properly (non-disjunction), so one gamete ends up with two copies of chromosome 21 instead of one. When such a gamete fuses with a normal gamete at fertilisation, the resulting zygote has three copies of chromosome 21 (trisomy 21) instead of the usual two, giving a total of 47 chromosomes instead of 46.
