During the follicular phase, rising FSH stimulates follicle growth and estrogen secretion in the ovary, while estrogen causes the uterine endometrium to regenerate and thicken in preparation for possible implantation.

The follicular phase marks the first half of the menstrual cycle, beginning on day one (the first day of menstruation) and continuing until ovulation, typically lasting about 10–14 days in a 28-day cycle. This phase is named for the dominant event in the ovary: the maturation of ovarian follicles. But the changes are coordinated across both the ovary and the uterus, orchestrated by a carefully timed hormonal dialogue between the hypothalamus, anterior pituitary, and the ovary itself.

Events in the Ovary

The anterior pituitary secretes follicle-stimulating hormone (FSH) at the start of the cycle. FSH does exactly what its name suggests: it stimulates a cohort of primary follicles in the ovary to grow. These follicles are fluid-filled structures, each containing an immature egg (oocyte) surrounded by layers of granulosa cells.

As the follicles develop, the granulosa cells proliferate and the follicles enlarge. Critically, these growing follicles begin to secrete estrogen in increasing amounts. Usually, one follicle outpaces the others and becomes the dominant or Graafian follicle by the end of the follicular phase—this is the one that will eventually release its egg at ovulation. The other follicles degenerate, a process called atresia.

Note

The rising estrogen levels exert negative feedback on FSH secretion initially, which helps ensure that typically only one follicle matures fully each cycle. Later, high estrogen triggers positive feedback, leading to the LH surge that causes ovulation.

Events in the Uterus

While the ovary is busy maturing follicles, the uterus is recovering and rebuilding. Remember that menstruation has just stripped away the functional layer of the endometrium (the uterine lining).
