The kidney's ability to concentrate or dilute urine comes from the counter current mechanism, the coordinated opposing flow of fluid through Henle's loop and blood through the vasa recta.

One of the defining abilities of the mammalian kidney is producing urine that is far more concentrated than the body's own fluids, and this rests on a specific structural arrangement rather than on any single reabsorption step.

  • In Henle's loop, filtrate flows down the descending limb and back up the ascending limb — two opposite directions running side by side, forming a counter current.
  • The vasa recta, the capillary network running alongside the loop, carries blood in a similarly opposing pattern.
  • Because these two structures lie close together and both maintain counter current flow, they progressively build up the osmolarity of the medullary interstitium, climbing from about 300 milliosmoles per litre in the cortex to roughly 1200 milliosmoles per litre in the inner medulla, driven mainly by sodium chloride and urea. …
