Step 1 — Structures involved.
The mechanism operates through two U-shaped, parallel structures that lie side by side in the renal medulla, running in opposite directions to each other:
- The loop of Henle (of juxtamedullary nephrons), carrying the glomerular filtrate.
- The vasa recta, a capillary network that runs alongside the loop of Henle, carrying blood.
Step 2 — How the gradient is built (loop of Henle).
- The descending limb of the loop of Henle is permeable to water but relatively impermeable to salts (Na⁺, Cl⁻); as filtrate flows down into the medulla, water moves out into the surrounding interstitium, and the filtrate becomes progressively more concentrated.
- The ascending limb is impermeable to water but actively transports NaCl out into the medullary interstitium (thick ascending limb), so the filtrate becomes progressively more dilute as it moves up.
- Because flow in the two limbs is in opposite directions ("countercurrent"), the salt pumped out by the ascending limb keeps adding to — and reinforcing — an already concentrated interstitium created by the descending limb, producing a graded osmotic gradient that increases from the cortex towards the inner medulla (roughly 300 mOsm/L near the cortex to about 1200 mOsm/L deep in the medulla in humans).
