The swollen necks Ketki observes are signs of goitre, caused by iodine-deficient hypothyroidism; if the same deficiency occurs during pregnancy it can cause cretinism in the developing baby.

a. A visibly swollen neck in a population is the classic external sign of an enlarged thyroid gland, a condition called goitre.

b. Goitre results from hypothyroidism, a shortage of thyroid hormone. The thyroid's hormones, thyroxine (T4) and triiodothyronine (T3), both require iodine from the diet to be made at their normal rate. In hilly and inland regions, dietary iodine is often naturally scarce, so the gland cannot synthesise enough hormone. In response, the gland enlarges, trying to compensate, and this enlargement produces the visible swelling in the neck. …
