- AB blood group individuals are universal recipients: Blood group is determined by the antigens (A, B) present on the RBC surface and the antibodies present in the plasma. A person of blood group AB has both A and B antigens on the RBC membrane, but — because the immune system does not make antibodies against its own antigens — the plasma of an AB individual contains neither anti-A nor anti-B antibodies. Since agglutination (clumping) of donor RBCs happens only when the recipient's plasma antibodies react with antigens on the donor's RBCs, an AB individual's antibody-free plasma cannot agglutinate RBCs of any ABO group (A, B, AB or O). This is why AB individuals can receive blood from all four ABO groups and are called universal recipients.
