CO2 released by tissues is carried in blood mainly as bicarbonate ions (largest share), some bound to haemoglobin as carbaminohaemoglobin, and a small amount simply dissolved in plasma.

CO2 produced by cellular respiration diffuses from tissues into the blood and is transported to the lungs by three mechanisms:

  1. As bicarbonate ions (~70%): CO2 diffuses into red blood cells, where the enzyme carbonic anhydrase rapidly catalyses its combination with water to form carbonic acid (H2CO3), which dissociates into H+ and bicarbonate (HCO3-). Most of the HCO3- then diffuses out into the plasma (in exchange for chloride ions entering the RBC — the 'chloride shift'), and is carried to the lungs, where the reverse reaction regenerates CO2 for exhalation.
  2. As carbaminohaemoglobin (~20-23%): CO2 combines directly and reversibly with the amino groups of the globin part of haemoglobin (not the same binding site as O2) to form carbaminohaemoglobin, which is transported to the lungs and releases CO2 there. …
