The carbylamine (isocyanide) test is a characteristic reaction of only primary amines: heating with CHCl3 and alcoholic KOH produces an extremely foul-smelling isocyanide, and this test is negative for secondary and tertiary amines.

When a primary amine (R-NH2 or Ar-NH2) is heated with chloroform (CHCl3) and alcoholic potassium hydroxide (KOH), it is converted into an isocyanide (carbylamine), which has a distinctive, extremely unpleasant/offensive smell:

R-NH2 + CHCl3 + 3KOH (alc., heat) -> R-NC (isocyanide, foul smell) + 3KCl + 3H2O

Mechanism outline: alcoholic KOH first generates dichlorocarbene (:CCl2) from CHCl3; this reacts with the lone pair of the primary amine's nitrogen, and subsequent loss of 2 HCl (removed by KOH) gives the isocyanide.
