The cornea lacks its own blood supply, so it is largely hidden from the recipient's immune system, making cornea transplants unusually free from rejection.

Organ and tissue transplant rejection normally occurs because the recipient's immune system recognises the donor tissue's cell-surface antigens (which differ from the recipient's own) as foreign, and immune cells (along with antibodies) delivered via blood vessels attack the graft.

The cornea, however, is a transparent, avascular tissue — it has no blood vessels of its own and obtains its nutrients by diffusion from the aqueous humour and tear fluid instead. Because immune cells and antibodies normally reach tissues via the bloodstream, the absence of blood vessels in the cornea means the recipient's immune system has very limited access to the transplanted corneal tissue, greatly reducing the chance of an immune attack and rejection.
