The Reproductive and Child Health Care (RCH) programme is a comprehensive government initiative launched in 1997 that integrated maternal health, child survival, family planning, and reproductive tract infection services to address India's reproductive health challenges through awareness, accessibility, and quality care.

The reproductive health of a population determines not just individual well-being but the demographic and developmental trajectory of an entire nation. Recognizing this, the Government of India launched the Reproductive and Child Health Care (RCH) programme in 1997, building upon and expanding the earlier family welfare programmes. The shift was significant: from a narrow focus on population control through family planning, the approach broadened to encompass the entire spectrum of reproductive health needs across the life cycle.

The Vision and Scope

The RCH programme was designed as an integrated package of services. It brought together maternal health, child survival and health, family planning, and the management of reproductive tract infections (RTIs) and sexually transmitted infections (STIs) under one umbrella. This holistic approach acknowledged that reproductive health is not merely about contraception or safe delivery—it encompasses informed choice, access to quality services, prevention and treatment of infections, and the health of both mother and child from conception through early childhood.

The programme aimed to achieve several interconnected goals:

  • Reduce maternal mortality and morbidity by ensuring skilled attendance at birth, emergency obstetric care, and antenatal and postnatal services
  • Lower infant and child mortality through immunization, management of childhood illnesses, and promotion of breastfeeding and nutrition
  • Expand access to contraceptive choices and family planning information, enabling couples to make informed decisions about spacing and limiting births
  • Prevent and treat RTIs and STIs, including raising awareness about HIV/AIDS
  • Address adolescent reproductive health through education and counseling services

Implementation Strategy

The programme operated through a decentralized structure, with states and districts given flexibility to tailor interventions to local needs. Key components of implementation included:

Service Delivery Improvements: The programme strengthened the existing health infrastructure—from sub-centers and primary health centers in rural areas to urban health posts—equipping them with trained personnel, essential drugs, and equipment for safe deliveries and newborn care. Auxiliary Nurse Midwives (ANMs) and Accredited Social Health Activists (ASHAs) became the frontline workers, bringing services closer to communities.

Awareness and Education: Mass media campaigns, community meetings, and interpersonal communication were deployed to spread awareness about reproductive health issues, the importance of institutional deliveries, immunization schedules, contraceptive methods, and the dangers of RTIs and STIs.
