Gene therapy corrects a genetic disorder by delivering a normal copy of the defective gene into the patient's cells. In ADA deficiency, a functional ADA gene is introduced into the patient's lymphocytes (via a viral vector) and returned to the body; repeated because mature lymphocytes are short-lived. A permanent cure needs correction of bone-marrow/early cells.

Concept

Gene therapy is the correction of a genetic defect by inserting a normal, functional gene into an individual (or embryo) to replace or compensate for the non-functional gene — treating the cause of a hereditary disease rather than just its symptoms.

Example — Adenosine Deaminase (ADA) deficiency

  • ADA deficiency is caused by the deletion/malfunction of the gene for the enzyme adenosine deaminase. Without ADA, the immune system fails, causing Severe Combined Immuno-Deficiency (SCID).
  • In some cases it is treated by bone-marrow transplant or by injecting the enzyme, but these are not permanent cures.

The gene-therapy procedure (first done in 1990):

  1. Lymphocytes are removed from the patient's blood.
  2. These are grown in culture outside the body.
  3. A functional ADA gene (cDNA) is introduced into these lymphocytes using a retroviral vector.
  4. The genetically-corrected lymphocytes are returned to the patient's body.
  5. Because these mature lymphocytes are not long-lived, the process must be repeated periodically with fresh treated cells.

Towards a permanent cure

If the functional ADA gene could be introduced into the bone-marrow stem cells (or into cells at an early embryonic stage) that produce lymphocytes for life, the disorder could be cured permanently.
