The LH surge is the sharp, mid-cycle spike in luteinising hormone that triggers ovulation — the release of a mature egg from the ovary — and is the single most critical hormonal event of the menstrual cycle.

To understand why the LH surge matters, you first need to see the menstrual cycle as a carefully orchestrated hormonal conversation between the brain and the ovaries. The hypothalamus sends out GnRH (gonadotropin-releasing hormone) in pulses, which tells the anterior pituitary to release two key hormones: FSH (follicle-stimulating hormone) and LH (luteinising hormone). During the first half of the cycle — the follicular phase — FSH does the heavy lifting, nurturing a handful of ovarian follicles until one becomes dominant. That dominant follicle, in turn, produces rising levels of oestrogen.

Now here is where it gets interesting. For most of the follicular phase, oestrogen exerts a negative feedback on the pituitary, keeping LH and FSH in check. But when oestrogen crosses a certain threshold — typically around day 12–14 of a 28-day cycle — its effect flips to positive feedback. This sudden reversal causes the pituitary to release a massive, abrupt surge of LH, and to a lesser extent FSH. This is the LH surge.

The LH surge is the trigger that makes ovulation happen. Without it, the mature egg would remain trapped inside the follicle and no pregnancy could occur.

What does the LH surge actually do? It acts on the dominant follicle in the ovary, causing it to swell rapidly and release enzymes that weaken its wall. Within about 24–36 hours of the surge’s onset, the follicle ruptures and the secondary oocyte (the egg) is expelled into the fallopian tube. This is ovulation. The surge also initiates the conversion of the ruptured follicle into the corpus luteum — a temporary endocrine gland that secretes progesterone to prepare the uterus for implantation. …
