Erythroblastosis fetalis is Rh-incompatibility haemolytic disease of the newborn, caused by maternal anti-Rh antibodies attacking fetal Rh-positive RBCs.

The Rh factor is an antigen present on the RBC surface of most people (Rh-positive); those lacking it are Rh-negative. Problems arise when an Rh-negative mother carries an Rh-positive fetus (the fetus having inherited the Rh antigen from an Rh-positive father):

  1. During the first pregnancy, fetal Rh-positive RBCs can leak into the mother's blood, usually at delivery. The mother's immune system, having never encountered the Rh antigen before, gets sensitised and produces anti-Rh antibodies — but only after birth, so the first baby is generally unaffected.
  2. In a subsequent pregnancy with another Rh-positive fetus, the mother's pre-formed anti-Rh antibodies (IgG, which can cross the placenta) enter the fetal circulation and destroy the fetus's RBCs, causing severe anaemia in the fetus. …
