Background - the Rh factor: Human RBCs may or may not carry the Rh antigen on their surface. Individuals with the antigen are Rh-positive (Rh+); those without it are Rh-negative (Rh-).
Mechanism:
- An Rh-negative mother carries an Rh-positive foetus (having inherited the Rh+ allele from an Rh+ father).
- During a normal pregnancy, the mother's and foetus's blood are largely kept separate by the placenta, so there is usually no problem during the first Rh+ pregnancy.
- However, at the time of delivery (or due to any placental damage during pregnancy, miscarriage, or abortion), a small amount of the Rh-positive foetal blood can leak into the mother's circulation.
- Since the mother's immune system does not recognise the Rh antigen as "self", it gets sensitised and produces anti-Rh antibodies against it.
- In a subsequent pregnancy with another Rh-positive foetus, these pre-formed maternal anti-Rh antibodies (being small IgG molecules) can cross the placenta and enter the foetal circulation. …
