Erythroblastosis foetalis is a disease of the foetus/newborn arising from Rh blood-group incompatibility between the mother and the foetus.
Mechanism:
- The problem arises when the mother is Rh-negative (no Rh antigen) and the foetus is Rh-positive (has Rh antigen, inherited from an Rh-positive father).
- Normally the maternal and foetal blood do not mix during pregnancy because they are separated by the placenta. However, during the delivery of the first Rh-positive child, some foetal Rh-positive blood leaks into the mother's circulation.
- In response, the mother's immune system produces anti-Rh antibodies.
- During a subsequent pregnancy, if the foetus is again Rh-positive, these maternal anti-Rh antibodies cross the placenta into the foetal blood and destroy the foetal red blood cells (haemolysis).
- This causes anaemia and jaundice in the baby (the destroyed RBCs release bilirubin). In severe cases it can be fatal to the foetus. …
