Breathing works on a pressure gradient created by rhythmic changes in thoracic volume, following Boyle's law (pressure and volume are inversely related).

Inspiration (breathing in):

  1. The diaphragm contracts and flattens, increasing the vertical dimension of the thoracic cavity.
  2. The external intercostal muscles contract, lifting the ribs and sternum upward and outward, increasing the antero-posterior and lateral dimensions.
  3. Together these increase thoracic (and hence pulmonary) volume.
  4. By Boyle's law, an increase in volume decreases the pulmonary (intra-pulmonary) pressure to slightly below atmospheric pressure.
  5. Air moves from the region of higher pressure (atmosphere) to lower pressure (lungs), so air flows into the lungs through the nasal passage, pharynx, larynx, trachea, bronchi and bronchioles into the alveoli.

Expiration (breathing out):

  1. Under normal, quiet breathing, expiration is passive — the diaphragm and external intercostal muscles simply relax.
  2. The diaphragm domes back up and the rib cage falls back to its resting position, decreasing thoracic volume.
