Surrogacy is an arrangement in which one woman carries a pregnancy on behalf of another person, who becomes the newborn's legal parent after birth. In practice this is usually combined with IVF: embryos are created in a laboratory from the intended parents' (or donors') gametes and then transferred into the surrogate mother's uterus to develop.

On the male side, the chapter singles out azoospermia — the complete absence of sperm in the ejaculated semen, confirmed on at least two separate occasions, and found in roughly 1% of the population — as a condition requiring a specific surgical solution rather than a standard ART procedure. Micro-Testicular Sperm Extraction (TESE) addresses it directly: through a small midline incision in the scrotum, one or both testicles are exposed, and under a microscope, the seminiferous tubules are examined and a small amount of testicular tissue from areas of active sperm production is carefully removed.
