WHO's simplified clinical definition of AIDS (developed for settings without confirmatory lab testing) relies on a combination of major and minor clinical signs rather than a laboratory HIV test alone.

Major signs:

  • Weight loss of more than 10% of body weight.
  • Chronic diarrhoea persisting for more than one month.
  • Prolonged fever (either intermittent or constant) for more than one month.

Minor signs:

  • Persistent cough for more than one month.
  • Generalised, itchy (pruritic) dermatitis.
  • Recurrent herpes zoster (shingles).
  • Oropharyngeal candidiasis (oral thrush).
  • Chronic, progressive or disseminated herpes simplex infection.
  • Generalised lymphadenopathy (swelling of lymph nodes at more than one site).

Clinical case definition: the presence of at least two major signs together with at least one minor sign, in a patient with no other known cause of immunosuppression (such as cancer, severe malnutrition or long-term steroid use), is taken as clinically suggestive of AIDS.
