Hernia and appendicitis are two important abdominal surgical conditions - a mechanical protrusion of an organ through a weak wall in one case, and an inflamed, potentially rupturing appendix in the other - each with characteristic causes, symptoms and treatment.

HERNIA

Definition: a hernia is the abnormal protrusion of an organ or part of an organ (most commonly a loop of intestine) through a weak point or defect in the muscular/connective-tissue wall of the cavity that normally contains it, most often the abdominal wall.

Types:

  • Inguinal hernia - the most common type, where abdominal contents push through the inguinal canal in the groin.
  • Umbilical hernia - protrusion through a weak area near the navel, common in infants and in adults with raised abdominal pressure.
  • Hiatus hernia - part of the stomach pushes up through the diaphragm's oesophageal opening into the chest cavity.
  • Femoral and incisional hernias - through the femoral canal, or through a weakened previous surgical scar, respectively.

Causes: a combination of an inherently weak or congenitally defective area in the abdominal wall and factors that raise intra-abdominal pressure, such as heavy lifting, chronic coughing, straining during defecation or urination, obesity, pregnancy, and ageing (which weakens muscle tone).

Symptoms: a soft swelling or bulge that becomes more prominent on standing, coughing or straining, and that can often be pushed back in (reducible) when lying down; a dragging sensation or pain, especially with exertion. If the blood supply to the protruding tissue is cut off (strangulated hernia), it becomes a surgical emergency with severe pain, vomiting and, if untreated, tissue death.

Treatment: small, asymptomatic hernias may be managed conservatively with a supportive truss/belt, but the definitive treatment is surgical repair (herniorrhaphy, or hernioplasty using a synthetic mesh to reinforce the weak area); strangulated hernias require emergency surgery.

APPENDICITIS

Definition: appendicitis is the inflammation of the vermiform appendix, a small, finger-like, blind-ended tube that projects from the caecum at the beginning of the large intestine.

Cause: it typically begins with obstruction of the narrow appendiceal lumen, most often by a hardened piece of stool (faecolith), or occasionally by swollen lymphoid tissue or a foreign body; the trapped secretions allow bacteria to multiply, causing infection, inflammation and swelling of the appendix wall, which can progress to gangrene and rupture (perforation) if not treated promptly.
