When an Rh-negative mother carries an Rh-positive foetus, foetal Rh-positive red blood cells can enter the maternal circulation (usually at delivery), triggering the mother's immune system to produce anti-Rh antibodies. These maternal antibodies pose no threat to the current pregnancy (sensitisation happens too late), but in a subsequent Rh-positive pregnancy they can cross the placenta and destroy the foetal red blood cells, causing severe haemolytic anaemia/erythroblastosis foetalis.
