Adolescence — the transitional stage of life between childhood and adulthood — is singled out in this chapter as a period of particular vulnerability to the abuse of tobacco, drugs and alcohol, collectively called TDA abuse, and understanding why, and how it can be prevented, is treated as an essential part of biology and human welfare.

Why adolescence is a vulnerable period

Several factors converge during adolescence to increase the risk of TDA abuse. There is natural curiosity about new experiences at this stage of life. Peer pressure is intense, as adolescents are strongly influenced by the behaviour and expectations of their friend groups and may feel compelled to try a substance to fit in or appear mature. There is often a desire for novel or thrilling experience, which substances can seem to offer. Adolescents also face significant stress, whether academic (examination pressure, career expectations) or personal (family or relationship difficulties), and may turn to substances as an unhealthy coping mechanism. Compounding all of this, adolescence is a stage where the capacity for mature judgement and impulse control is still developing, while in some settings substances remain easily available.

Tobacco and its harm

Tobacco use, whether smoked (cigarettes, bidis, cigars) or in smokeless form, delivers nicotine and tar deep into the respiratory tract. Chronic use paralyses and eventually destroys the protective cilia lining the airway, causes chronic bronchitis and emphysema (destroying alveolar walls and severely reducing the lungs' capacity to exchange oxygen), and its carcinogenic tar content is a major cause of lung and oral cancers — harm that begins accumulating from the very age at which many people first experiment with tobacco, often during adolescence.

What constitutes drug abuse

Drug abuse is defined as the self-administration of a substance, at a dose or frequency not warranted by any genuine medical need, purely for its mood-altering or pleasurable psychological effect, in a manner that damages the user's physical health, mental health or social functioning — as distinct from the legitimate medicinal use of the same substances under medical supervision.

The substances commonly abused, and their sources

Several commonly abused substances trace back to specific plant sources. Smack (crude heroin) is derived from the opium poppy, Papaver somniferum, being a processed derivative of morphine extracted from opium latex. Cocaine is obtained from the leaves of the coca plant, Erythroxylum coca. These same plants, together with Cannabis sativa (source of marijuana/charas/ganja/hashish-type products), are the classic hallucinogenic plants whose extracts alter perception, mood and consciousness and are widely abused for these effects.

Substances banned in competitive sport

Certain substances are specifically prohibited for athletes because they confer an unfair advantage or endanger health: cannabinoids are banned for their psychoactive and performance-altering/impairing effects, and anabolic steroids — synthetic derivatives of the male hormone testosterone — are banned because some athletes abuse them to artificially build muscle mass and strength, at the cost of serious long-term health risks such as hormonal imbalance and organ damage.

Medicinally valuable drugs that are also abused

It is important to recognise that several of these substance classes are also genuinely valuable medicines: opioids are indispensable for the relief of severe pain, cannabinoids have some recognised medical applications, and cocaine was historically used as a local anaesthetic. It is not the substance itself but its non-medical, self-administered use purely for psychoactive effect, outside proper medical supervision, that constitutes abuse.
